Pottery Barn Kids "Emerson Crib" - with mattress and toddler bed conversion kit set
Picture shown is fully assembled, but it has since been taken apart so that it could be removed from the room.
While dismantling, one of the screws stripped so I left that section assembled so as not to damage. Should still be able to fit it in your vehicle.
